Have you considered the fact that it is pretty much a universal feature
of email clients that once you click 'send' the compose window goes
away? i.e. duplicate messages (and for what it's worth I only received
one copy of Mark's reply) are generally NOT a product of someone actually
stupidly clicking Send on the same message over and over, but a glitch
in either their mail client, or an intermediate mail server handling
the message.

Email on the Internet generally travels as

sender -> sender's outgoing server -> ... -> recipient's incoming
server -> recipient's inbox.

The ... can be zero or more intermediate mail servers. At any point
along this chain, there is the possibility (fortunately rare) that
the receiving agent thinks the transaction has succeeded and dutifully
forwards the message along the chain, yet the sending agent thinks the
transaction *failed* and therefore tries it again.
