Thanks very much for taking a look at these machines. I certainly wasn't going to ask to have them added to the database without some confirmation first.

There was no bingo card on the screen shot I was sent, maybe because the photo did not show the sides and top beyond the display of the cards and paytable (BTW which was 99.42% 15/10/4/4 DW).

Maybe this will help some, Jean. It appears that 6% was withheld until June 12, 2012. Thereafter it was 4%:

"Pursuant to R.C. 5747.063, if a person's winnings at a casino facility are such that the IRS requires reporting on form W-2G or 1042-S ($1,200 or more not reduced by the wager for slot winnings; more than $5,000 reduced by the wager or buy-in for table win; or $600 or more if the winnings are more than 300 times the amount wagered or are subject to federal income tax withholding), the casino operator shall deduct and withhold Ohio income tax from the person's winnings at a rate of four percent (4%) of the amount won. (Please note that, prior to June 12, 2012, each casino operator was required to withhold Ohio income tax at a rate of six percent (6%) for those people meeting the thresholds mentioned above.)"

Jean

You may want to consider sending an e mail each of the casinos and ask them
what the withholding rates on W2G's are for Ohio and respective City. I
would think that they would respond to your question in a timely manner. I
can' imagine that the WH rate would be 6% unless they are combining the city
and state tax WH.

your W2-G's from each of those cities, and be sure to add whether you are a
resident of that particular city.
