For a while, my problem was just the opposite. My mailer said I'd get
$5 if I played $20, but I had $5 in free play already loaded.

That's mail fraud, you should report it:
United States Postal Inspection Service

Always get something in writing so you have something to take to gaming enforcement. A verbal agreement with a casino is worthless. And don't get in the habit of leaving comp at a casino assuming they will faithfully bank it for you, they won't, you're just encouraging them to steal it, which they will do. Casinos are far worse than used car dealers, if you keep that in mind you'll probably do ok, and make sure to put the gaming enforcement numbers on your smart phone as well:
Nevada Gaming Control Board : Enforcement Division

In the past, I've received something in the mail saying something like play $20 and receive $5 in free slot play. Of course, one would ignore something so insignificant of an offer.
But, for June, I received an email with the following;

I guess I was hoping for the best, some free slot play without playing. It turns out, it's the same offer as previously. Don't make the trip, it's a waste of time.

I've reported this here before, but I'd like to repeat that I went too
long without playing on my card at the Rampart and my points expired.
I asked about having them reinstated and I was told that if I lived up
to the level of my previous play for a year, they'd "be happy" to
reinstate them, which I did, only to be told that they "couldn't"
reinstate them (carefully not using the word "wouldn't," implying that
they didn't have $200 to give me).
