As pyiddy said, any number of programs can be used to figure out which hands are played differently. (I'd add WinPoker to that list--it's the one I used before making the comments below.)
One significant change for a 5000-unit jackpot is that you hit all 3-card RF's rather than keep a high pair, unless your 3-card RF has both an Ace and a Ten. (If the jackpot is 5500, then even the AT 3-card RF's are held.)
You'll also be able to stop worrying about when to hold KTs vs holding just the K. No more penalty cards.

The confusion lies in the fact that the W-2G forms used by Cleveland, Cincinnati and Columbus do not have a separate box for local income tax withholding. I have no idea why they don't as I have W-2Gs from one casino in IN that has a box for local withholding even though there is no local withholding there. I've also seen versions of W2-G forms on the IRS website that also have box for local withholding.
Each of the 3 OH casinos I've visited seem to do it differently.
Cleveland just dumps the state and local both into Box 14 at a 6% rate (4$ state and 2% city) and makes no mention anywhere on the form that this is a combination of both.
Columbus dumps the state (4%) into Box 14, but then has this footer under the signature line that says "Local Tax Withheld xx.xx" which is at the 2.5% rate for Columbus. Your payout is the jackpot amount minus both he Box 14 and the local line item withholding.
Cincinnati dumps both state and local into Box 14, They have a footer under the signature line that says "Box 14 includes 4% state and 2.1% city tax withheld.
Someone else can verify Toledo, but it wouldn't surprise me to see a fourth way or reporting the local tax in OH W2-G.
One other oddity about Cincinnati is that they pay you on the spot from the slot attendant's pouch in cash and don't give you an actual W-2G at the time of your jackpot. You get this pink W-2G guest receipt and then the actual W2-G document arrives in the mail 2-3 weeks later.

If you have software (VP for Winners, Frugal VP, Optimum VP, VP strategy
Master or a number of others) you can use that. (Will be alot of work for
100% accuracy.)
Michael Shackleford's wizardofodds.com has a program on that site that
creates 100% accurate strategy charts for more than 95% of all vp games.
Those charts can be hard to read.
vpgenius.com covers about 99% of all games but their charts are not as
accurate.
In previous replies some many years ago, I have explained how to use
software to calculate the point at which plays change. (Rather than trial
and error.)

If the picture was cropped that tight, it's still very likely a Class II game given current Alabama law. When I goofed around on Class II machines in Alabama, many of the "bingo cards" were where you expect the player's club display to be, so if he just took a pic of the game monitor, it would be likely cut off.
