John,
re "loss of use", first check your benefits under your auto ins and any applicable credit card rental coverage. You'll likely find you're fully covered (particularly under card coverage with the like of Citi, etc.)
A few years ago, on this subject, I recounted my experience with a leisure rental that was involved in an accident while sitting in traffic. The repair was close to $10k.
Because we were car owners at the time, I sough coverage through the credit card used for the charge. It happened that this was a secondary issuer, now defunct. The repairs were pocked up, but I didn't have loss of use coverage, and Avis sough another $450 from me. On the phone they were very nice and with modest prompting offered to accept 50%. Good enough for me (but I now make sure I use a card that covers loss of use charges under its rental coverage.
