I use an AX gold card with the $24.95 per rental option for up to 42 days per rental for primary coverage for the rental car damages. Eg. no claim on my personal auto policy for crap like we are talking about. The card fee is $150 per year and I rent over 20 times per year. $650 per year  and no hassle(which seems to come up at least once every year). Even if you do not rent as often this is probably a cheap for only one incident. Alamo and Budget are really good to deal with, Enterprise(which is owned by Alamo) and Payless and any of the lesser companies are suspect and be sure to take cell phone pictures of condition of any little ding or scratch and document, with duplicates signed by an agent before  leaving the lot.
AX is very clear-NO fleet utilization report no payment. Maybe  big insurance with deep pockets put car rental companies in their place?

I've done a little digging and it seems that rental car companies don't have to show fleet utilization or even actual loss to claim loss of use fees. That is the scary part. They can charge you the full rental price even though they aren't renting the vehicle and incurring those expenses ( wear and tear, depreciation, cleaning, etc). So, loss of use becomes the biggest profit center for them. Scary, scary, scary.
