Maximizing the expectation for the play is not always maxEV strategy. You illustrated that with your suggested strategy for flush attack, which was a min-cost-flush strategy, given certain conditions as you stated. The conditions are always important and can dramatically change the optimal strategy. The problem is that I don't think this is what most people want to hear, they want to hear that there is always one perfect strategy for all conditions, not the dreaded "it depends". But optimal gambling is all about the "it depends" conditions.
