One of the six major Teams busted out due to bad math skills. They were a poser team run by an ex-manger of the Red Team. Their leader Tommy was not even close to a "true pro" and they got what they deserved.
The Team that failed was nicknamed The Rock Opera, because it's leader was named Tommy.
Of the five major real progressive teams I interacted with, none of them failed.
I went inactive in the late 90's, so there could be more modern stories I'm not privy to.

There's a lot of stuff that happens that doesn't make the light of day, for various reasons. But hey, as they say, out of sight, out of mind, nobody seen nuthin', lol.
