There has been a known issue for years with Dream Card and 4s.
If playing any bonus game that rewards for quad 2-4s and dealt a handful of nonsense that includes a 4 as the lowest card, the machine will pair a higher card instead of the 4 every time.
It correctly recognizes that a pair of 2s is superior to a pair of 10s, same with a pair of 3s. But if you are dealt 468T off suit it doesn't recognize that pairing the 4 is superior when suggesting a DreamCard.
I've put around $100k through the DreamCard machines at Rincon and it makes that error every single time. I've never noticed any other error.

Both I and the player next to me noticed that, very occasionally, the "Dream Card" offered by the machine had a lower EV than some other card that we could select by using the "Change Dream Card" option.
