It's nice to know that they aren't just picking on me. I filed my return in January and I am still waiting for my refund.

In order to apply for a refund of the 30% withheld amount, you must file a 1040NR return, and attach to it 1042-S form(s) documenting the gross winnings and withheld amounts. I don't think I would want to chance making that filing without my correct name on the 1042-S.
Incidentally, it is now more than seven and a half months since I filed to recoup 2014 withholdings! Previously, this process took six to eight weeks. My thoughts on this are unprintable.
